Transplant Diabetes Program

The Transplant Diabetes Program at Stanford Hospital & Clinics is a national and local award winning program that addresses diabetes through a multidisciplinary approach. 
 

Transplant physicians, diabetes educators, endocrinologists, nurse transplant coordinators, registered dieticians and pharmacists work together to coordinate comprehensive quality care that is accessible to the patient.

 

Transplant candidates and recipients with diabetes, and their family members are encouraged to participate in this program.

Transplant Diabetes Management Overview

A successful transplant requires teamwork. We believe that the person with diabetes and their family are the most important members of the diabetes team.

 

Our experienced interdisciplinary group specializes in working with people throughout the transplant process. Diabetes affects each person differently and we help create individualized treatment programs that accommodate a patient's needs and optimize their health.
