The Transplant Diabetes Program at Stanford addresses diabetes through multidisciplinary approach. Transplant physicians, diabetes educators, endocrinologists, nurse transplant coordinate, registered dieticians and pharmacists work together to corrdinate comprehensive quality care that is accessible to the patient. Transplant candidates and recipients with diabetes and their family members are encouraged to participate in this program.

A successful transplant requires teamwork. We believe that the person with diabetes and their family are the most important members of the diabetes team. Our experienced interdisciplinary group specializes in working with people throughout the transplant process. Diabetes affects each person differently, because of this we help create individualized treatment programs that accommodate the patient's needs and optimize their health.

Transplant Diabetes Class
Learn how good blood sugar control improves transplant outcomes. This one-time class is for individuals with diabetes, pre- and post-transplant. The class is team taught by a diabetes educator, registered dietian, pharmacists and nurse transplant coordinator. Basic principals of diabetes management are reviewed along with the interrelationship of blood glucose management and transplantation. Family members and friends are encouraged to attend.

APPOINTMENT INFORMATION
Weekly Walk-in Appointments
The walk-in discussion sessions provide a place for patients to problem solve their diabetes-related issues. Staffed with a registered dietitian, pharmacist and diabetes educator who coordinate with the patient's transplant program. Group leaders can help review diet, blood glucose and medication records, blood glucose meter education, nutrition consultation, and medication on an as-needed basis.

Individual Appointments
Provided on Thursday during transplant clinic appointments and / or Transplant Diabetes Endocrine appointment. These appointments offer individuals a focused time and training and are extremely helpful for patients new to diabetes or who are recently experiencing changes in their diabetes management. We will help provide timely diabetes education and support that will coordinate with your scheduled appointment.

Diabetes Data Logs
While in the Transplant process your blood glucose control and nutritional needs may change. In anticipation of these changes we have made available a blood glucose log and food diary log that you or your family can down load, complete and bring to your appointment so we can monitor your progress while at home. The information you provide will help us better accommodate your needs. Please feel free to write down any questions and or concerns you may have before your appointment.
