Goal

To acquire the clinical skills and experience necessary to ensure therapeutic and cost effective pharmaceutical treatment on an inpatient medicine service

Objectives

·   Apply appropriate pharmaceutical care principles in the management of patients on the medicine service

·   Become an active and productive member of the medicine team

·   Provide accurate and concise drug information to other healthcare providers

·   Act as a drug information resource to patients                                                                               

Activities

·   Participate in daily work and attending rounds

·   Daily patient monitoring to ensure efficacy and safety of drug therapy

·   Provide patient education and discharge counseling on medications

·   Documentation of clinical interventions in the computer or chart

·   Prepare and present an in-service to the medicine team

·   Meet with preceptor on a regular basis to discuss patients
