Advance Health Care Directives

New Patient Statements

Good news for our patients. We are proud to announce a redesigned look and easier-to-read format for our hospital and physician billing (www.patientbilling.stanfordhospital.com) statements. View a sample of the new statements .

What is an Advance Health Care Directive?

An Advance Health Care Directive is a form which will ensure that your health care wishes are known and considered if for any reason you become unable to speak for yourself. Completing the Advance Directive will allow you, under California law, to appoint another person to be your health care "agent".

 

The "agent" will be able to make decisions about your medical care if you become unable to make these decisions. The Advance Directive will also allow you to write down detailed instructions about your treatments. Your doctor and your agent must follow the instructions detailed in the directive.

Where can I get an Advance Health Care Directive form?

For those patients who have been admitted to the hospital, advance care directives are available through the Patient Representation program. You may also pick up an advance directive from the Health Library at no charge.
