Vial of Life
|
Vial of Life is a medical information storage kit for use in the home. The kit enables emergency responders, such as firefighters, paramedics and police, in Santa Clara and San Mateo Counties to quickly locate helpful medical history information.
The Vial of Life kit is being distributed to seniors in Santa Clara and San Mateo counties through the Guest Services department, Lifeline program, Stanford Health Library, healthcare organizations, senior centers and other community agencies. Please direct questions to VialofLife@stanfordmed.org

The kit contains:
- Medical Information Form
- Vial to store the medical form and Advance Directive
NOTE: The vial should be stored on the top shelf in the door of the refrigerator
- Two stickers:
- One sticker is placed on the outside of the vial.
- The second sticker is to be placed on the outside of the refrigerator in the upper right corner.
