Post-transplant diabetes mellitus (PTDM) may occur in as many as one third of transplant recipients who were not previously diabetic. Diabetes is a leading risk factor which significantly complicates patient quality of life and graft survival.
The Transplant Diabetes Program at Stanford Hospital and Clinics is nationally recognized as the first formal program in the nation to address diabetes in the pre- and post-transplant population.
A multidisciplinary team of health care professionals who specialize in transplantation and diabetes will present an overview of a patient-centered approach to education, management, and monitoring.
Program Highlights:
-
Strategies to decrease:
- length of stay
- incidence of diabetes-related readmissions
- diabetes-related complications
-
Screening skills for diabetes in the transplant population
-
Techniques to empower patients
-
Existing interdisciplinary resources to optimize transplant diabetes outcomes
-
Treatment targets based on international guidelines for new onset transplant diabetes
-
Current Medicare and insurance coverage plans regarding diabetes screening, care, and supplies
